Welcome to our Forms section. Throughout the year, we will post any forms that need to be filled out for our various Youth Events. Just find the appropriate form below and download it straight from here. You can also find all our forms at the Resource Tables outside Fellowship Hall as well as in the Youth Area!
WCPC General Release Form
This form covers our regular, weekly programming. This includes for instance, weekly Youth meetings, events on church property, small field trips, etc. We require that this form be filled out once a year for each student.

WCPC Special Event Release
This form is required for special events throughout the year, such as our Spring Retreat, Concerts, and major Off-Site Events for instance. It’s mostly the same as our General Form. Please fill in the Event and Date name.

Fall Retreat Forms
The following form is required, along with a $25 deposit, to secure students’ spots for the Jr. High Laurelville and Sr. High J-Town Fall Retreats! Please check the appropriate Event/Date boxes on the form. You can also download a packing list for your convenience!

Picture/Media Requests
We take tons of photos during our various events. Chances are that you, either as parents or students, love to keep photos as well. We would be more than happy to share our photos with you. We can provide photo CD’s of specific events, time periods, even filter by photos of people (such as all photos of your student).
If you would like to request a photo CD, please contact Mike at bartonm@wexfordcpc.org to discuss your specific request. We simply ask for a $1 donation to cover the cost of ordering CD’s!
